What is Aftercare?

Aftercare is the process of moving through and on from care and offers support to young people in the care system to move on into independence.

To help young people move on into independence and should happen in consultation with the young person and the process can involve assessments, care plans and reviews being carried out before the young person reaches the age of 18years.

Why do I need to plan for leaving care?

The purpose of the preparation and planning for young people to live independently is to achieve the best outcomes for young people who have lived in foster care, residential care or any other care setting.

Aftercare is a commitment to young people in care as care leavers to support them through programmes and consistent support services to prepare the young person to live out on their own.

How long will I receive aftercare support?

The support available is up to 21 years of age or 23 years if in full time education.

What is the aim of Aftercare support?

Aftercare support aims to improve outcomes for young people leaving care by;

  • Young person learning life and social skills
  • Young person being resilient and have coping skills
  • Encouraging young people to engage in employment, training or further education
  • Having suitable accommodation where young people can become involved in these communities
  • Having friends and other people to rely on
